The parturition research is the major topic in obstetrics. The important discoveries have been made and new concepts have been formulated during last 3 decades. But parturition mechamsm is not yet achieved. It is a continuous matter of concern
what
substances are synthesized in the fetal membrane during parturition and what intermediators exist between signals from fetus and uterotonins.
It is postulated that endothelin-1 initiate the labor pain, and EGF acts as a mediator to endothelin-1. The tissue was obtained from umbilical cord, amnion, chorion, placenta and decidua in each groups of full term vaginal delivery(n=14),
full-term
cesarean section(n=10), preterm delivery(n=9), pregnancy-induced hypertension(n=7), Immunohistochemical localization of endothelin-1 and EGFR were performed and analyzed by scoring system and Student t test, Mann-Whiteney U test.
The endothelin-1 and EGFR were expressed in all tissues of paracrine arm during pregnancy and labor. There were significant correlation between endothelin-1 and EGFR. EGFR were expressed more intensely in vaginal delivery than cesarean delivery,
and
those are also more intensely in fullterm delivery than preterm labor. There was no significant correlation with fetal weight, duration of labor, PROM, histologic chorioamnionitis in expression of endothelin-1 and EGFR.
These data suggest that endothelin-1 and EGFR have a paracrine effect on the fetal membrane and acts as a uterine contractant at the initiation of parturition. Endothelin-1 and EGFR are important substances which act as a mediator in parturition
and
which have a significant correlation in paracrine arm.
